How to check GLP-1 coverage with Blue Cross Blue Shield

Blue Cross Blue Shield is a national brand used by independent local companies. The Blue Cross Blue Shield Association does not keep your member file or run a single national formulary.
Formularies differ by employer, state, Medicare vs. commercial, and PBM carve-out. The only reliable answer is the number on your ID card plus a written prior-auth decision.

How prior authorization usually works here
- Identify which local BCBS company issued your plan (Anthem/Elevance, Highmark, HCSC, Blue Shield of California, and others are separate insurers). The first three letters of your member ID often map to that company on bcbs.com.
- Call the number on your ID card and ask for pharmacy benefits / formulary, not medical-only customer service.
- Ask whether GLP-1 coverage is decided by the local Blues plan, a carved-out PBM, or your employer. Many Blues plans use a PBM (the vendor varies by company — do not assume it is the same one your coworker has).
- Prior authorization is almost always submitted by the prescriber through that PBM’s portal or CoverMyMeds. Ask which portal your local plan uses.
- Because each Blues company writes its own medical policy, a headline about “BCBS covering Ozempic” in another state does not answer your question. Stay on the phone until they confirm your contract.
What to ask on the call
- Which local Blue Cross or Blue Shield company is my plan, and which PBM runs the drug list?
- Is [drug name] covered on my formulary for [diagnosis], at what tier, and with what prior-auth criteria?
- Does my employer carve out pharmacy benefits to a different vendor?
- What BMI, diagnosis, or step-therapy rules apply to this specific contract?
- If denied, how do I appeal with this local company (not the national association)?
