How to check GLP-1 coverage with Kaiser Permanente
Process guidance only — this page does not say whether Kaiser Permanente covers Ozempic, Wegovy, Mounjaro, or Zepbound on your plan.
Kaiser Permanente is a group of regional health plans and medical groups, not one national insurance contract. Formularies, pharmacies, and phone numbers differ by region. The numbers below are only those published on a region’s dedicated member-services page; for every other region, use the number on your ID card.
Formularies differ by employer, state, Medicare vs. commercial, and PBM carve-out. The only reliable answer is the number on your ID card plus a written prior-auth decision.

How prior authorization usually works here
- Because Kaiser is integrated, coverage questions often go through your Kaiser pharmacy or your Kaiser clinician rather than an outside PBM. Start with the Member Services number for your region, or the pharmacy number on your prescription label.
- Look up your region’s drug formulary on kp.org (each region publishes its own). A Southern California formulary does not speak for Mid-Atlantic or any other Kaiser region.
- If a GLP-1 requires review, ask your Kaiser prescriber or the pharmacy to initiate it. In some Kaiser regions, “prior authorization” is an internal clinician review rather than a member-submitted insurance form.
- Ask whether your plan is a Kaiser HMO, a Kaiser added-choice/PPO-style product, Medicare Senior Advantage, or Medicaid/Medi-Cal — the pharmacy rules are not the same.
- If the drug is non-formulary in your region, ask Member Services how to request a non-formulary exception or (for Medicare) a coverage determination. Do not assume another Kaiser region’s policy applies.
What to ask on the call
- Which Kaiser region and which plan type am I on (HMO, added-choice, Medicare, Medicaid)?
- Is [drug name] on my region’s formulary for [diagnosis]?
- If it needs review, does my Kaiser clinician submit that internally, or is there a separate pharmacy coverage-determination form?
- What is the cost at a Kaiser pharmacy vs. mail-order, and are there quantity limits?
- If it is non-formulary, what is the exception or grievance process for my region?
