A drug can appear on a Part D formulary and still not be payable for every diagnosis. Medicare still treats drugs used only for weight loss differently from drugs used for an FDA-approved non-weight-loss indication (for example, type 2 diabetes, cardiovascular risk reduction, or sleep apnea). Use the tables below as a map of which formularies list the product and at what tier, then confirm the indication and your cost with your plan or Medicare.gov Plan Finder.

If you need it for weight management

CMS runs a separate, time-limited demonstration — the Medicare GLP-1 Bridge (July 1, 2026 through December 31, 2027). CMS says it operates outside the Part D benefit: eligible people pay a $50 monthly copay, and the $50 does not count toward the Part D deductible or TrOOP. Covered products listed by Medicare.gov are Foundayo, Wegovy (injection or tablet), and Zepbound KwikPen only. If you already get a GLP-1 through your Part D plan, CMS says you stay on the plan — you are not eligible for the Bridge. How to confirm your own plan in one call

  1. Have your Medicare card and plan name ready (for example, a standalone PDP or a Medicare Advantage plan with drug coverage).
  2. Search Medicare.gov Plan Finder for your ZIP code and the exact drug name, or call 1-800-MEDICARE.
  3. Ask: “Is this covered for my diagnosis, what tier, and is prior authorization required?” A formulary listing for diabetes does not automatically mean coverage for obesity.
  4. If it is for weight management only, ask whether you should use the Medicare GLP-1 Bridge instead of (or in addition to) a Part D coverage determination. CMS: medicare.gov/coverage/weight-loss-drugs.
