  • A prescriber decides whether to prescribe and submit the request.
  • Prior authorization is required. The documented decision window is 72 hours.
  • The documented copay is $50 per month for a one-month supply.
  • The program runs July 1, 2026 through December 31, 2027.

  • Foundayo (tablets)
  • Wegovy (injection and tablets)
  • Zepbound (KwikPen only; single-dose pens and vials NOT covered)
