Coverage varies. Cigna's Wegovy injection prior authorization policy (review date 06/17/2026) covers Wegovy with prior authorization: an 8-month first approval after at least 3 months of behavioral modification and diet, a BMI of 30 or higher (or 27 or higher with a comorbidity), and 1-year continuation with 5 percent weight loss. Express Scripts' 2026 National Preferred Formulary lists Wegovy as the preferred weight-loss product, but Cigna's own policy states a customer's plan document supersedes the policy and may exclude it. Cigna's individual-market National Preferred 3-tier list does not include Wegovy.

Approval criteria
- Adults 18 and older: a trial of behavioral modification and dietary restriction for at least 3 months, with Wegovy used alongside a reduced-calorie diet.
- Baseline BMI of 30 or higher, or 27 or higher with at least one listed comorbidity: hypertension, type 2 diabetes, dyslipidemia, obstructive sleep apnea, cardiovascular disease, knee osteoarthritis, asthma, COPD, MASLD or NAFLD, polycystic ovarian syndrome, or coronary artery disease.

Renewal criteria
- Has lost at least 5 percent of baseline body weight.
- Still used with behavioral modification and a reduced-calorie diet.
- Continuation approval lasts 1 year.

Exclusions and limits
- Using Wegovy together with another GLP-1 receptor agonist or a GLP-1/GIP agonist is not recommended and is listed as not covered.
- Any use not listed in the policy is considered not medically necessary.
