Coverage varies. Zepbound is listed Tier 2 with PA, QL and the '++' flag on the 2026 Optum Rx Premium Formulary, meaning coverage depends on the member's benefit design. UHC's weight loss PA notification (August 2026, effective 10/1/2026) says weight-loss drugs are an optional program for clients that elected coverage. Zepbound criteria cover chronic weight management (BMI 30+, or 27+ with a weight-related comorbidity) and obstructive sleep apnea. Reauthorization needs about 5% weight loss from baseline plus continued lifestyle modification, with initial approvals of 3 to 6 months.
Applies to: Commercial plans only where the employer or plan sponsor elected the optional weight-loss drug program; otherwise excluded (benefit design)
